Forms4Us, a low-cost online service available exclusively through Benefit Software Inc., offers a convenient web site document storage and retrieval service for your employee benefits, HR and other types of documents. HR staff time can be significantly reduced with this service. You'll avoid waste because you will no longer need to print copies that usually wind up being thrown out when the form changes. Expensive storing and shipping of forms is a thing of the past with our new online document service!

When a document is added, updated or replaced, simply send an original copy of the new one to us and we will add and index it for you. When a new document is added or an old one is changed, we can send an e-mail notification to each employee that includes a link to the Forms4Us Website to let them know the new form is available. If any of your employees do not yet have e-mail access, we can print and mail announcements to them or send you payroll stuffers to inform them about the new documents. You may also wish to have us set up computer kiosks in an employee lunchroom, your HR department, or other convenient locations where employees can access the documents.

Wide Range of Options
The
Forms4Us service offers a wide range of options. For example, some documents will be viewed by the employee and, if needed, printed. An example of this might be an employee handbook. Other forms, such as beneficiary designation forms, may be printed, completed and signed by the employee, then mailed or hand delivered. COBRA applications and 401(k) enrollment forms can be designed as interactive forms that the employee can fill out online with a confirmation copy going to the employee. You may also choose to have e-mail forms for less important issues such as change of address.

The Forms4Us service also offers database forms that feed the data entered by the employee into a database application. An example of a database form is an employee survey or questionnaire. Responses to the survey form are transferred to a database where the results are tabulated and viewed by authorized personnel.

Another option you may select is to have us tell you which employees have not reviewed certain forms. For example, you may want to know who has read the Summary Plan Description for your 401(k) plan. We can provide you with a list of employees who have not yet downloaded the document and we can also send them an e-mail reminder to do so.

You may also have your documents set up so that employees can sign them with a simple, safe and widely accepted hand-written digital signature application. This option requires the use of a TouchPad device that plugs into the computer. We can supply these at very low cost. The Forms4Us service may be subscribed to alone or in conjunction with the Fringe Facts Online Benefits and Compensation Communication Service.

Health Insurance Application Form (PDF) The insurance carrier requires this form to be signed by the employee. The employee will download, print, and then fill out and sign the form.

Continuing Education Reimbursement Form (PDF) The employee will fill this out online and print it.

Employee Benefit Enrollment and Change Form This form is completed online. Data is sent by E-Mail message to the HR department with a confirmation copy to the employee. Optionally, the data may be fed directly into an HRIS or other system.
